Whenever an intervention is being delivered it is important that its delivery and effectiveness are monitored and evaluated. This template has been designed to help you do this.
Whenever an intervention is being delivered it is important that its delivery and effectiveness are monitored and evaluated. This template has been designed to help you do this.
The first part of the template is where you will record the background information of the intervention which will provide you with a useful summary. By entering the entry and exit data you will be able to identify the amount of progress made. If you are using this template in conjunction with the provision map template, you may wish to attach a ‘grade’ to various amounts of progress e.g. 0-1 months could be given a 1, 2-4 months a 2, and so on. These scores can be entered on the provision map to make it easier to evaluate the effectiveness of the provision. Alternatively you may just wish to use a simple Yes or No.
The Number of Sessions column will make your evaluation of an intervention more accurate. So, if progress for a young person has been minimal for example it is important to check how many sessions they have attended; it could be low attendance that is the issue rather than an ineffective intervention.
The main section of the template, as well as acting as a register, also encourages the adult delivering to be reflective and to consider what might need to happen next time. If being used by a Teaching Assistant, it could also form useful evidence for performance management.
If used electronically, the template will ‘grow’ as it is used. If you decide to use a printed version you may wish to make the rows higher so that there is room to add meaningful comments.
